Snoring and obstructive sleep apnea syndrome (OSAS) are between the most actual medical problems. Different surgical procedures are proposed for the treatment of snoring and OSAS. Most of them directed to stiffen the soft palate. Pillar palatal implant system is one of the modern non-invasive surgical treatments. This review aims detail information of the mild to moderate obstructive sleep apnea and snoring treatment using Pillar soft palate implants. In the article we described technique of Pillar procedure, patients’ selection, complications and comparative analysis of the soft palate operations. Also short-term and long-term outcomes of treatment are reviewed. According clinical studies results, Pillar palatal implants are effective in the treatment of snoring and mild to moderate OSAS. Patients treated with palatal implants demonstrate reducing of snoring intensity in average of 45 percent and achievement of short-term bed partner snoring satisfaction in average of 82 percent of cases. Almost half of the mild to moderate OSAS patients achieve the decrease in apnea/ hypopnea index in average of 41 percent, also the same percent of patients show objective treatment “success”. Moreover, palatal implant procedure provides significantly improvement in patients’ quality of life. We can conclude that pillar procedure is simple, non-invasive with low morbidity and complications rate. Careful selection of the patients and avoidance of general anesthesia could improve success. Pillar palatal implants can be recommended as a first line treatment for carefully selected patients with snoring and mild to moderate obstructive sleep apnea syndrome.
