Karotidinė paraganglioma. Diagnostika, gydymo taktika (klinikiniai atvejai)

Paraganglioma priklauso retai pasitaikančių neuroendokrininių galvos ir kaklo navikų grupei. Šie navikai istoriškai vadinami "glomus tumorais", "chemodektomomis". Mes pristatome du klinikinius atvejus, kai navikai buvo operuoti be ir su priešoperacine embolizacija.
Objective: To present the cases histories of 60 and 40 year old patients with carotid paraganglioma, investigated in Klaipëda University Hospital, Maxillofacial departament in 2007 year, to review etiopathogenesis of this tumor, clinical and imaging findigs. Materials and Methods: Cases were diagnosed after clinical examination, computed tomography and conventional angiography. Results: Computed tomography can show vascularised carotid paraganglioma, particularly after intravenous contrast medium injection. Conventional angiography can evaluate relation between tumor and carotid artery. This method is a „golden standard“ for evaluating invasion of paraganglioma to vessels. Carotid paraganglioma has to be differentiated with wide range of disorders: pseudotumors, injures of neck vessels, orofacial and neck infection, benign and malignant tumors of the neck, metastatic lymph nodes. Biopsy and aspiration are contraindicated for diagnosis of paraganglioma of this localisation. Early surgery is necessary to prevent tumor invasion and metastasis.
